Frequently Asked Question

Can one person receive support for more than one condition?

Absolutely. Many of the people we support have overlapping needs – for example, a learning disability alongside autism, or an acquired brain injury with associated mental health difficulties. Our care plans are built around the whole person, not a single diagnosis, so we blend the relevant expertise and approaches into one coordinated support package.

Every care plan is different because it is written around the individual. However, plans commonly cover personal care, medication management, daily living skills (cooking, budgeting, household tasks), community access and social activities, emotional and psychological support, and coordination with GPs, therapists, and other healthcare professionals. Plans are regularly reviewed and updated as needs change.

PBS is an evidence-based framework that focuses on understanding the reasons behind behaviours that challenge, rather than simply trying to manage them. It involves assessing triggers, teaching new skills, and adjusting the environment to reduce distress. We use PBS across all our care packages because it leads to better outcomes for the individual and reduces the need for restrictive practices. Our approach is aligned with the national Restraint Reduction Network.

The Oliver McGowan Mandatory Training on Learning Disability and Autism is the government- endorsed training standard for health and social care staff in England. It ensures our team has a thorough understanding of learning disabilities and autism so they can deliver informed, respectful care. We conduct regular care plan reviews and respond quickly when needs change. If a person’s condition deteriorates or improves, we work with the individual, their family, and the wider multidisciplinary team to reassess and adjust the care package. This might mean increasing or decreasing support hours, bringing in additional specialist input, or updating goals and outcomes.

Yes. Our acquired brain injury and mental health packages are specifically designed to support individuals stepping down from hospital, rehabilitation, or secure settings into community-based living. We work closely with discharge teams to plan a smooth transition, including pre-move visits, phased introductions to the new home environment, and ongoing clinical coordination.
